The wheelchair is held down by belt-style restraints with retractors that are supposed to lock, much like a seat belt locks. On these vans, the retractors may fail to lock, so the wheelchair is not really tied down. If it is not secured, the chair and the person in it can roll or tip while the van is moving, braking, or turning.
These are vans converted by Rollx for wheelchair use. The straps that hold a wheelchair use retractors, which work like a seat belt: they pull in slack and then lock so the chair cannot roll. On these vans, the QRT-Deluxe and QRT-Max retractors may fail to lock. If they do not lock, the wheelchair and the person in it can shift or slide while the van is moving, which raises the risk of injury. Rollx and the restraint maker will check the retractors and replace them if needed.
These vehicles have a Panoramic View Monitor system that shows what is behind and around you. A software error can make the backup camera image freeze or go completely blank when you shift into reverse. That means you may not see a child, a pet or an object behind the vehicle. Federal rules require a working rear view. A dealer will update the parking assist software at no cost; it is a software fix, not a part replacement.

A Toyota dealer will tighten the bolts for free. The repair is just tightening the bolts already in the van, so no new parts are needed.
These Toyota and Lexus vehicles have a sensor in the front passenger seat that tells the airbag system whether someone is sitting there. A short circuit can develop inside that sensor. If it does, the front passenger airbag may not inflate during a crash, so whoever is riding in that seat could be hurt worse than they otherwise would be. The dealer will inspect the sensor and replace it if needed, free of charge. Owner letters are being mailed in stages.

Your 2022 Sienna was built with seating for eight, and the second-row seat belts on the outside seats may have the wrong webbing guide — a small part the belt strap slides through. In a crash, the belt tightens fast, and the wrong guide can damage the strap and tear it. A torn belt cannot hold you the way it is supposed to.
